Clinical Study of oral electrolyte solution and lactose free formula for nonbacterial acute gastroenteritis in children




Abstract
The clinical studies were performed under treatment with oral electrolyte solution and lactose free formula in 30 cases with moderate to severe nonbacterial acute gastroenteritis in children who had admitted to the pediatric department of Korea Univ. Guro hospital during 6 months period from Nov. 1984 to Apr. 1985.
The patients were divided into two groups. Group A(30 cases) received oral rehydration solution (Pedialyte¢çRS) and lactose free formula(Isomil¢ç) and group B(30 cases) was given intravenous fluid and milk based formula.
The results were obtained as follows,
1) In group A, 29 patients (96.7%) were treated successfully but only one case failed. This Patient also suffered from a urinary tract infection. Twenty five patients in group B were also successfully treated (83.3%) but we have deemed that this high total cure rate might have been influenced by 7 cases in which continuous IV therapy was provided for by reason of the patients conditions. This was contrary to our initial protocol that was to convert to full strength milk formula after 48 hrs of initial IV fluid therapy.
2) On the second day of treatment, the frequency of diarrhea in group B was slightly increased which was considered as secondary lactose intolerance as a result of milk based formula.
3) Vomiting was noticed in 21 cases(70%) in group A and 25 cases(83%) in group B and was improved within the first day of treatment. For oral rehydration therapy, vomiting was not a big problem.
4) After the beginning of therapy, hyponatremia was not noticed in all patients.
In Summury, there are some exceptions to rehydration therapy, we have considered that the oral rehydration soltion(Pedialyte¢çRS) and lactose free formula(Isomil¢ç) were effective on mild, moderate and some cases of severe nonbacterial acute gastroenteritis in children caused by Rota virus or other viral agents, and this therapy offers some advantage over the usual treatment.
The use of O.R.S. obviates IV needle and a soy isolate formula does not cause secondary lactose intolerance.
